The Limitations Paragraph: Honest Without Sinking the Paper
The limitations section paper is often written last, when the authors are tired and the conclusion already feels settled. That is backwards: the paragraph is where a paper tells the reader exactly how far its result can travel. A useful limitations paragraph is neither a defence brief nor a catalogue of every imperfection. It is an argument about interpretive boundaries.
Treat limitations as threats to interpretation
A limitation matters when it could change the meaning of the main result, not merely because it happened during the project. The ICMJE recommendations ask authors to state limitations and to avoid conclusions that their data do not adequately support. That makes the section part of the evidence chain, rather than a ritual performed before submission. Name the mechanism: selection may limit generalisability, an unmeasured confounder may distort an association, and incomplete follow-up may bias an outcome estimate.
Vague phrases such as “the sample was small” do little work. Small for which estimate, and with what consequence? A sharper version says that imprecision prevents a confident conclusion about a clinically important difference. The reader can then see both the weakness and the claim that remains defensible.
Select what could alter the conclusion
Start with the study’s central claim and ask what would most plausibly overturn it. For an observational study, that may be confounding, missing outcome data, or a sample drawn from a setting unlike the intended population. The STROBE statement is a helpful prompt for cohort, case-control, and cross-sectional work because it makes authors account for design, participants, variables, bias, and study size. For an intervention study, incomplete intervention description can itself prevent readers from judging whether an apparent effect can be reproduced; TIDieR was designed for that problem.
Do not pad the list with minor inconveniences. A limitation that has no credible route to changing interpretation distracts from the real risk. The strongest paragraph is selective enough that a reviewer can see the authors have already asked the awkward questions.
Pair each limitation with its real mitigation
Mitigation is evidence that reduces a concern; it is not a sentence that makes the concern disappear. A prespecified analysis may reduce the worry that a subgroup result was chased after the data were visible, but it does not repair an underpowered subgroup. Sensitivity analyses can show how much an inference depends on an assumption, but they cannot prove the assumption true. Say what was done, then state the residual uncertainty.
This distinction protects the paper from two bad habits. Defensive writing denies a weakness with “however” and moves on. Self-sabotage lists weaknesses without explaining what the design, analysis, or cautious conclusion did to contain them. The middle ground is more credible because it lets the reader inspect the remaining uncertainty.
Put the most consequential limit first
Order is judgement. Begin with the limitation that most affects internal validity or the practical reach of the result; follow with narrower concerns. A buried limitation reads as though the authors discovered it only after drafting the conclusion. A front-loaded limitation says the conclusion has already been calibrated around it.
That approach also improves the surrounding Discussion. It also prevents the final section from becoming a second Results narrative rather than an interpretation.
A limitation paragraph worth keeping
For each candidate limitation, write four linked parts: the design feature, the direction or type of bias it might introduce, the mitigation actually used, and the boundary it leaves on the conclusion. Then read the conclusion again. If the conclusion would need to change after taking the limitation seriously, change the conclusion rather than softening the limitation.

Build this check into the final results-to-submission sprint, before the conclusion hardens around an unsupported implication. Honesty is not a concession here. It is the reason a cautious claim can still be useful.
